By filling out the following application form, parties agree as follows:

• Company agrees to participate in the preparation of, and consents to Meridian's publication of a case study. The case study may include Company logos, non-confidential information, quotation and the names/images of Company's employees or other authorized representatives. Meridian may display Company's name and logo on its website.
  
  
• Meridian will obtain Company's approval of the final case study content prior to its public release. Company agrees to review the final content for factual accuracy and to prevent the release of any confidential information.
  
  
• Company authorizes Meridian to use Company's logo in logo montages in connection with marketing and promotional activities including launch events, promotional materials, slide presentations and the like solely in conjunction with Company's use of Meridian solutions.
  
  
• Meridian will have the right to use, reference and display the final case study, press release and/or testimonial advertising describing Company's use of Meridian products and services as follows: by publication on Meridian's internal and public websites; by publication in any and all media; in printed copies distributed to Meridian employees and customers; in printed copies distributed at Meridian sponsored or co-sponsored events; in unedited excerpts included in speeches, slides, brochures and other marketing collateral materials, and as reference when communicating with analysts, press and the general public. Meridian agrees that the content of the case study, press release and/or testimonial advertisement will not be altered without prior written consent from Company.
